This link will take you to the Intoxilyzer Examination Site hosted by the Nebraska Department of Health and Human Services. Upon successful completion, you will be emailed a certificate. When submitting Attachment 9 to NDHHS, you can indicate more than one device for which you want your Class B Permit. You must attach supporting documentation that you passed the online course(s) testing with your Attachment 9.
