This link will take you to the Intoxilyzer Examination Site hosted by the Nebraska Department of Health and Human Services. Upon successful completion, you will receive your certificate via email. When submitting Attachment 9 to NDHHS, you can indicate more than one device for which you want your Class B Permit. You must attach supporting documentation that you passed the online course(s) testing with your Attachment 9.
