This link will take you to the Intoxilyzer Examination Site hosted by the Nebraska Department of Health and Human Services.  This is only a test of your knowledge regarding Intoxilyzers, NOT Training.  Training on Intoxilyzers is on your own.  Intoxilyzer offers in-person training courses.  More information is available here.

Upon passing this online exam, you will receive an email with your certificate of completion.  When submitting Attachment 9 to NDHHS, you can indicate more than one device for which you want your Class B Permit (See picture below).  You must attach all supporting documentation that you passed the online course(s) testing with your Attachment 9.  Example: You take the online course for DataMaster offered through NLETC's training platform Moodle.  You will receive your certificate of completion via email.  You then take the online Intoxilyzer test and pass successfully.  You will receive a certificate via email for that.   You must submit both certificates of completion with Attachment 9 to NDHHS, and check both boxes (DataMaster & Intoxilizer) on Attachment 9.
